Appointment

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Most often, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation first and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any CT imaging from your dentist on CD or flash drive if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to dental treatment or if you require antibiotics before standard dental care (i.e. for joint replacements, heart conditions, or immunocompromised patients)

X-Rays

If your previous dentist has taken recent x-rays (within a few weeks), you may request that they forward them to our office. New x-rays are always taken at the consultation appointment to ensure we know the current condition of your teeth.  In many cases we will require 3-D CBCT (Cone-Beam Computed Tomography) imaging to give us highly detailed information about your tooth.  This is especially necessary in complex cases such as retreatment and surgical cases.
